Two apps version: how to synchronise or transfer data from the old one to the other?

I just realise that my app Map My Tracks on iPhone unable to do its job as best as I thought (I used it since 2013 and never change it). But, because I used it rarely (about 3-5 times every year) so I had no idea if there was a new version launched on Apple Store. Since than, I change my iPhone model 3 times to the new one (4, 5, and SE).
Today, I tried to search on App Store and I found out that my Map My Tracks store on my iCloud other than on my iPhone SE, while it was there the old app still on my screen. I used the old one since last week last month and my history registered as well.
The old app was not work as normal as I want. I can not see my maps of all the last activities I made about the last 3 weeks. I can not take any picture or upload existed images from my library. I can not synchronise it with my web version either.
So, can I transfer my old history from the old app to the new one? Or how can I upload/synchronise the old app data to my web version?

If all your previous activities have been synced to Map My Tracks then you can download them back to your new phone by using the Recover option in the app Settings > Recover.
This will download all your activities that you have synced (backed up) to Map My Tracks.
